I give Great Bridge Free Will Baptist Church/Great Bridge Christian Academy permission for my child to take part in all church school activities, including bus trips, sports activities, and church sponsored trips away from the church premises.
I further agree to hold the church school and it's agent harmless for any liability to my child or any guardian or parent thereof because of any claims on behalf of my child against the church school or any agent thereof because of any injury to my child. Should legal action, for any reason be taken against Great Bridge Free Will Baptist Church/Great Bridge Christian Academy or any employee or agent thereof on my child's behalf, and the church school or its agent not be found at fault, I agree to pay any attorney fees, court fees, damages or other costs that Great Bridge Free Will Baptist Church/Great Bridge Christian Academy or it's agent should incur to defend itself against such action.
I understand that Great Bridge Free Will Baptist/Great Bridge Christian Academy does not carry any type of accident insurance. All injuries must be filed through each family's health insurance policy.
This Statement of Cooperation will be in effect for as long as my child(ren) (listed or others to be enrolled) attend this church school. I understand that should my marital status change, it would then be my responsibility to have an updated Statement of Cooperation signed and delivered to the church.

1. We believe in the plenary, verbal inspiration of the scriptures which guarantees their accuracy and establishes them as a man’s only authoritative source and guide for life and faith.
2. We believe in the in the One, True and Eternal God who exists equally in three persons: the Father, the Son and the Holy Spirit.
3. We believe in the deity, virgin birth, vicarious atonement, bodily resurrection, and the imminent return of Jesus Christ for His bride, the church.
4. We believe in the necessity of the new birth for all fallen man, which is a work of grace wrought by the Holy Spirit conditioned by a personal faith in Jesus Christ as one’s Lord and Savior.
5. We believe in the local church and the universal church . The local church is constituted of an assembly of born-again believers who assemble to preach the gospel, administer the ordinances,
and practice Christian discipline. The universal church is made of all born-again believers of all areas and ages.
In signing the application, I understand that:
1. My child will be trained in accordance with the above Statement of Faith
2. My cooperation is expected in: (a) Regular tuition payment; (b) Practical help; (c) Faithful prayer; (d) Involvement in fundraisers.
3. That tuition is paid bi-monthly on the 1st and 15th of each month, and penalties are enacted for late payment.
